First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options, before a treatment is agreed on and delivered. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

It is generally advisable that young children and patients with special needs are seen for a consultation visit prior to treatment to increase the likelihood that the treatment visit will be as comfortable as possible.


Please assist us by providing the following information at the time of your consultation:

A list of medications you are presently taking (both prescribed and unprescribed).

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.


IMPORTANT: a parent or guardian must accompany all patients under the age of 18 years of age at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., any allergies, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever or any other conditions that might have an implication on your dental treatment) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, or any other medication that might have an implication on your dental treatment)..
